Community Health Center Compliance

  • HRSA/BPHC Operational Site Visit (OSV) Compliance
    • Stay in compliance with HRSA’s 19 Program Requirements (PRs) throughout the year
    • Conduct internal monitoring and auditing by performing periodic audits
    • Manage and assign Program Requirements to staff to develop policies and supporting documents to meet HRSA Standards
    • Conduct mock-site audits periodically to ensure compliance using the Compliatric Platform
    • Provide feedback to staff members assigned to certain Program Requirements
  • Patient Safety and Risk Management Compliance
    • Mitigate or reduce risk by developing risk assessments for Health Center divisions (e.g. claims management, credentialing of providers, finance/billing, patient satisfaction, legal and contracts, etc.)
    • Document current activities and the impact/likelihood of any threats or vulnerabilities throughout the organization
    • Periodically review risk analysis/policies and procedures related to risk management and patient safety
    • Document and analyze complaints and near missed reported by Health Center staff, patients and vendors
    • Run trend analysis reports of events and near misses
    • Improve patient safety and minimize organizational losses
  • Policies and Procedures Tracker
    • Develop, edit and review Health Center policies and procedures
    • Keep all policies and procedures in one area
    • Delegate responsibilities to key staff responsible for developing, reviewing and finalizing Health Center policies
    • Assign policies to key management staff or CEO for review and approval
    • Assign policies to Board Members for review, approval and signature
    • Develop monthly, quarterly or annual reminders to update policies and procedures
  • Contracts, MOAs, MOUs, Referrals Arrangements and Vendors Tracking
    • Maintain all MOAs/MOUs, Contracts and Referral Arrangements in one area
    • Ensure compliance by reviewing Form 5A, Columns II and III – and update existing MOAs/MOUs or develop new agreements based on services provided
    • Manage all contracts with external vendors
    • Maintain all employees and physician contracts in one area
  • Incident Reporting
    • Report an incident immediately using the software
    • Assign incidents to supervisor/manager
    • Review and respond to incident
    • Develop key action steps/plan to resolve incident
    • Maintain a log of all incidents in one area to improve quality
    • Respond appropriately to incidents within the set timeframe
    • Ensure that reported incidents are resolved appropriately
    • Periodic reminders sent to assigned staff when incidents have not been completed within assigned timeframe
  • Credentialing
    • Maintain all providers’ credentialing and privileging documents in one area
    • Assign tasks to providers missing certain documents within their files
    • Stay up to date and receive reminders of providers credentialing and privileging renewal
    • Share provider credentialing and privileging files with Board Members for review and approval within Compliatric
  • Board Communication and Education
    • Develop Individual Board Committees and assign tasks to Board Members
    • Allows for open lines of communication among Board members/Board committees
    • Maintain all Board meeting minutes and documents in one area
    • Request signatures of minutes from Board Chair, Board Secretary and Board Committee Chairs
    • Review Board documents electronically prior to meetings or during meetings
    • Provide training and education to Board Members on roles, responsibilities, Health Center practices and standards (through educational materials and videos)
    • Maintain Board training manual and new member orientation process in one area
